Before Your Visit
On this page you'll find the link to our online new patient paperwork, complete with instructions. Please dedicate time to fill out your health history in full and return it via email within 24 hours so our doctors can begin reviewing your case.
If we don't have your paperwork 48 business hours before your appointment, we unfortunately won't be able to hold your appointment and a $75 non-refundable fee will apply.
To optimize your time with the doctor, please arrive 30 minutes before your scheduled appointment so we can perform preliminary testing.

Do you accept insurance?
We see patients with a wide array of insurances and our team will work with you to determine the most affordable option for the care you need. In many cases, functional medicine services and testing won't fall under insurance coverage — we do everything we can to make care affordable and available to everyone.
Do you work with patients outside of the Kentucky area?
Yes, we offer long-distance functional medicine consultations (national and international) via telephone or webcam for those who live outside our area. We've worked with clients across North America, Europe, and Australia.
How much will a functional medicine program cost?
We strive to create a comprehensive, affordable program — not just for the wealthy. Patients can expect to invest anywhere from $3,000–$10,000 depending on the complexity of their health challenge. See the Finances section below for a detailed breakdown.
Can I see a chiropractor if I am pregnant?
Yes, many pregnant women find that chiropractic adjustments improve the pregnancy experience and make delivery easier. Adjustments are adapted to the stage of pregnancy and each patient's unique needs.
Are adjustments safe?
Chiropractic care is widely recognized as one of the safest drug-free, non-invasive therapies available for most back and neck problems. Spinal adjustments are extremely safe when performed by a licensed chiropractor.
What is chiropractic care?
Chiropractic is a health-care profession focused on the spine and other joints of the body and their connection to the nervous system. A trained chiropractor identifies areas of hypomobility or loss of function and corrects them with the most appropriate technique for each patient.
What is functional medicine?
Functional medicine seeks to identify and address the root causes of disease, viewing the body as one integrated system rather than a collection of independent organs. Our doctors stay on the cutting edge of functional medicine research and testing to bring you the best in hope and healing.
Cancellation / late policy
If you're unable to make your scheduled appointment, we require 24 hours notice, otherwise the visit will be forfeited from your health investment plan. Functional medicine consultations require 72 hours advanced notice, otherwise the visit will be forfeited with no refund.
What should I bring on my first visit?
A list of all medications and supplements you're currently taking. You're also welcome to bring a friend or family member if you'd find that helpful.
Investment
Finances
We strive to be upfront about what each service may cost and whether it's billable to insurance, so you can make the best decision for you and your family. For services not covered by insurance, we've designed Health Investment Plans to offer significant savings and remove the stress of finances.
Services Billable to Insurance
Chiropractic and active care services are verified through your insurance, with financial responsibility discussed before any charges accrue. Care plans are also available when insurance isn't billed or care isn't deemed medically necessary. Some services may be billable to Medicare and Medicaid.
Functional Medicine Consultations
Consultations aren't included under insurance coverage. We accept cash, credit card, debit card, or checking account for services rendered.
Laboratory Testing
Lab work is based on the individual patient and varies in price, generally ranging $35–$975. Testing is recommended by the doctor based on patient needs, with pricing and insurance information reviewed with you before any test kit or blood work order is given. Consultations to review lab results are not included in the price of the lab test.
Consultations & Health Transformation Plans
New Patient Evaluation: $300–$425 (excludes lab testing). Follow-Up Consultations: $350. Health Transformation Plans: 7-visit plans starting at $2,100, 4-visit plans starting at $1,200. These plans do not include labs or supplements. We offer discounts for one-time payment and monthly payment plans.
